MOS Employee Benefits

Global Imaging Systems, Inc. offers its employees a comprehensive health plan including Medical, Dental and Vision as well as life insurance and disability coverage. You may select any combination of these benefits to suit the needs of you and your family.

Health plan participation begins on the 1st of the month following your hire date. You may participate in the Global Imaging Systems, Inc. 401(k) Retirement Plan immediately upon hire.

Health Plan Eligibility

Regular full-time employees who work 30 or more hours per week. Spouses, same-sex domestic partners and dependent children up to age 26 are considered eligible dependents. The medical plan is subject to employer primary coverage, which means, if you are hired on or after January 1, 2006, your spouse or same-sex domestic partner are considered ineligible for medical coverage under the Global Imaging medical plan.

Medical – Aetna, Anthem BCBS, Cigna (single administrator assigned by zip code) & Kaiser Permanente (in some locations)

You have the option of enrolling in a high deductible health plan (HDHP) accompanied by a Health Savings Account (HSA) that includes an employer contribution and wellness incentives.

Dental – Aetna or Cigna (single administrator assigned by zip code)

You have the option of enrolling in one of three dental plans, including a basic PPO plan, a Premium PPO plan and a DMO plan (DMO – in network ONLY). Enrollment is through Aetna or Cigna and is determined by your home Zip Code.

Vision – Vision Service Plan (VSP)

You may choose vision coverage with a network of participating providers offering vision services and materials at a greater benefit level. Your vision coverage with VSP will be locked-in for 2 calendar years.

Basic Life/AD&D - Aetna

Company provides term life insurance for eligible employees: one times your basic annual salary up to maximum of $300,000.

Supplemental Life - Aetna

Employee Supplemental, spousal supplemental, and child supplemental life insurance coverage is available that include certain minimums and maximums based on the employee enrollment level.

Short-Term Disability (STD) - Aetna

You may purchase optional basic STD or STD Buy Up insurance through Aetna. The benefit covers the first 26 weeks of disability. No EOI required if enrolled within 31 days of your benefits eligibility date. The weekly benefit is 60% of your salary up to maximum of $1,400.00 per week under the basic plan or can increase to 66 2/3% of your weekly earnings to a weekly maximum of $2,300.00 under the Buy Up plan.

Long-Term Disability (LTD) - Aetna

The company pays 100% of your basic LTD premium coverage. Benefits begins 120 days after disability starts. You will receive up to 60% of your monthly income loss to a maximum of $6,000 per month. An LTD Buy-Up option is offered that will increase coverage to 66 2/3% of your monthly income up to a maximum monthly income loss of $10,000. Buy-Up option is 100% employee paid.

Flexible Spending Account - BenefitWallet

If you choose to waive enrollment for medical coverage with Global Imaging, you have the option of setting aside pre-tax dollars for out-of
pocket medical, dental, prescription and vision expenses. If you have eligible childcare expenses, you have the option of setting aside pre-tax
dollars to cover those expenses.
Healthcare Spending Account (HCRA) – you may contribute up to $2,550 per year.
Dependent Care Spending Account (DCRA) – you may contribute up to $5,000 per year ($2,750 if Highly Compensated Employee).
DCRA does not cover reimbursement of dependent healthcare expenses.

Provides access to a nationwide network of attorneys for legal advice and document preparation.

Voluntary Benefits – Allstate & Aetna

You can choose from Accident Insurance, Critical Illness and Hospital Indemnity coverage and elect coverage for both you and your family. The policies assist with those unexpected events and help bridge the gap with out-of-pocket medical costs.

Global Imaging Systems Inc. 401(k) Retirement Plan – Prudential

Contribute pre-tax dollars to your retirement. Regular full-time and part-time employees age 21 and older can participate in the 401(k) plan. You are eligible to participate immediately upon hire. Global Imaging will match 75% of every dollar you contribute up to the first 6% of employee contributions per pay period beginning the first quarter following one year of service.
